The first GLP-1 pill for weight management gets the green light
An oral version of Wegovy was approved in the US, making it the first tablet GLP-1 medicine for chronic weight management. Getting a peptide through the gut is the engineering story.
What's new
FDA approved a once-daily 25 mg semaglutide tablet (oral Wegovy) for chronic weight management and to reduce cardiovascular risk in eligible adults.
Why it matters
A tablet removes needles and the cold chain, two barriers for many patients.
What's next
Launch pricing, supply volumes and how it competes with non-peptide pills such as orforglipron.
US regulators approved a once-daily tablet of semaglutide for chronic weight management, the first oral GLP-1 medicine for this use. The dose, 25 mg, is far higher than the weekly injection because only a small fraction of a swallowed peptide reaches the blood.
How a peptide survives the stomach
Peptides are normally broken down by stomach acid and digestive enzymes. The tablet includes an absorption enhancer called SNAC, which:
- Raises the local pH around the tablet to protect the peptide.
- Helps semaglutide cross the stomach lining for a short time.
That is why the dosing rules are strict: take it on an empty stomach with a small amount of water and wait before eating or taking other medicines.
The evidence
The OASIS 4 trial compared the 25 mg tablet with placebo in adults with obesity or overweight over 64 weeks. People who took it as directed lost weight in the mid-teens percent range, close to the weekly injection.
Trade-offs
| Weekly injection | Daily tablet | |
|---|---|---|
| Dose of peptide | 2.4 mg per week | 25 mg per day |
| Timing rules | Any time of day | Fasting, wait before food |
| Storage | Refrigerated before first use | Room temperature |
The tablet is less a new drug than a delivery achievement: getting a 31-amino-acid chain to survive a trip that normally destroys it.
